WORK PERMITS

FYI _ Below is  the list of documents that should be submitted to the State Employment Agency together with the work permit application:

Documents to be produced by the Individual seeking permit:

1. Foreign Passport (as issued in the country of  citizenship -notarized copy of every page)*
2. Individual Identification Code (issued by the Ukrainian Tax authorities on request) **
3. Copy of the diploma of the individual seeking permit verifying education credentials.*

Documents to be produced by the hiring entity:

4. Certificate of Incorporation of the hiring entity (notarized copy)***
5. Charter of the hiring entity (notarized copy)***
6. Minutes of the board meeting appointing the Executive Body of the hiring entity (notarized copy)***
7. Order (signed and sealed decision ) of the executive body of the hiring entity to hire the individual
seeking permit for the position upon receipt of the permit by the foreigner.***
8.  Official Application issued by the hiring entity for work permit for its future employee**
9.  List of all foreigners legally working for the hiring entity at the moment***
10. Draft Employment Agreement with the foreigner seeking permit**
11. Justification of the hiring needs in this particular foreigner**
12. Certificate from the Gov-t Employment Agency verifying that the hiring entity is registered with the
agency as a payer of the unemployment tax, and that the accrued tax has in fact been paid in full.***

13 Official fee in the amount of USD 35  should be paid to support work permit application.
